Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,258 per week

Overview Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in West Palm Beach, Florida. Responsibilities Provide physical therapy services to geriatric patients in a skilled nursing facility, including assessment, treatment planning, and therapeutic interventions to improve mobility and reduce fall risk. Document patient progress using electronic medical records. Collaborate with the healthcare team to support patient recovery and functional goals. Minimum Qualifications Bachelor's, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Florida Physical Therapy license required to start the assignment. BLS certification required from AHA or ARC. Experience in skilled nursing facility settings preferred. Experience treating geriatric populations and managing mobility and fall-risk programs preferred. Ability to document using electronic medical records is preferred.
