Registered Nurse, Home Health

Role: Home Health Registered Nurse (RN) providing in-home nursing care to patients in their own residences. Responsibilities: Conduct home visits to assist patients recovering from injury, illness, or surgery, develop and manage care plans, perform assessments, medication management, wound care, and patient education. Deliver personalized, one-on-one care to promote recovery and meet patient goals. Provide education to patients and their families to support ongoing health management. Qualifications & Skills: Active RN license, with preferred clinical experience; previous home health experience is valued but not required as training is provided. Ability to travel locally as needed.
