Home Health Registered Nurse

We are seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated home health team. As an RN in the home health setting, you will play a vital role in providing high-quality medical care and support to patients in the comfort of their own homes. Your primary responsibility will be to assess patients' health needs, develop individualized care plans, and deliver personalized care to promote optimal well-being.

Responsibilities:

1. Conduct comprehensive assessments of patients' health conditions, including physical, emotional, and social factors, to develop personalized care plans.
2. Collaborate with the healthcare team, including physicians, therapists, and caregivers, to ensure seamless coordination of care.
3. Administer medications, perform wound care, and provide other nursing interventions according to established protocols.
4. Monitor patients' progress and adjust care plans as necessary to meet their evolving needs.
5. Educate patients and their families about health management techniques, medication adherence, and self-care practices.
6. Advocate for patients' rights and preferences, ensuring they have a voice in their care decisions.
7. Maintain accurate and up-to-date patient records and documentation in compliance with regulations and agency policies.
8. Promote a safe and comfortable environment for patients while respecting their privacy and dignity.
9. Communicate effectively with patients, families, and other healthcare professionals to ensure continuity of care.

Qualifications:

• Valid and unrestricted RN license in the state of practice.
• Proven experience as an RN, preferably with experience in home health, community health, or a related field.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities to interact effectively with patients and their families.
• Ability to work independently, demonstrating self-motivation and organization.
• Basic computer skills for electronic health record documentation.
