About the Role As a Home Health Caregiver with Homewell of Southern PA, you will play a vital role in delivering exceptional care to our clients in their own homes. Home Health Caregiver responsibilities may include: Providing one-on-one care to clients, including assistance with daily living activities, medication management, and emotional support Developing and implementing individualized care plans tailored to each client's unique needs and goals Maintaining accurate and detailed records of client care, including progress notes and medication administration Collaborating with interdisciplinary team members, including nurses, therapists, and social workers, to ensure comprehensive care Demonstrating a strong commitment to client-centered care, dignity, and respect Requirements High school diploma or equivalent required (or in the process of obtaining) Previous experience in the healthcare industry , preferably in a home health or geriatric setting Strong communication and interpersonal skills Ability to work independently and as part of a team Reliable transportation and a valid driver's license (if applicable) What We Offer At Homewell of Southern PA, we are committed to our employees' growth and development.
