Job Summary

We are seeking a dedicated ICMS Case Manager to provide Intensive Case Management Services for individuals living in Permanent Supportive Housing in Los Angeles, CA. This direct hire opportunity is ideal for a compassionate social services professional who is passionate about helping vulnerable individuals achieve housing stability, access critical support services, and build long-term self-sufficiency.

The ICMS Case Manager will work closely with residents in a high-needs supportive housing environment, providing individualized support, resource connection, crisis response, and ongoing service coordination. This role offers the opportunity to make a meaningful community impact while working as part of a mission-driven nonprofit environment with supportive leadership, collaborative teamwork, and a strong commitment to client-centered care.

This is a great opportunity for candidates with experience in case management, homeless services, behavioral health, mental health support, housing navigation, or community outreach who want to grow their career in supportive housing and nonprofit client care.

Key Responsibilities

- Provide intensive case management services and ongoing resident support within a Permanent Supportive Housing setting.
- Conduct client assessments, identify service needs, and develop individualized service plans focused on housing retention and stability.
- Connect clients to housing resources, mental health services, healthcare providers, public benefits, and other community-based supports.
- Maintain accurate case notes, progress reports, service documentation, and compliance records.
- Support crisis intervention, conflict resolution, de-escalation, and client engagement using a trauma-informed approach.
- Facilitate community meetings, resident engagement activities, and collaboration with internal and external service partners.

Compensation and Benefits

- Direct hire opportunity.
- Pay range: $21 to $24 per hour.
- Location: Los Angeles, CA.
