Your Responsibilities

• Executing thorough patient assessments, organizing diagnostic evaluations, and crafting personalized care strategies
• Diagnosing and addressing acute, episodic, or chronic health conditions in cooperation with physicians or independently, as per skills and credentials
• Informing patients and families on wellness practices, preventive measures, and effective disease management
• Prescribing medications and therapies as per your scope of practice
• Engaging in patient follow-ups and holistic chronic care management
• Working collaboratively with diverse teams to provide seamless and comprehensive care
• Meticulously documenting within electronic medical records (EMR)

Education & Licensing

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Required Credentials

• Active Nurse Practitioner license in California
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent specialized credential
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, or the capability to secure these within 90 days of starting
