What You'll Do
- Deliver skilled physical therapy services in patients' homes
- Assess mobility, strength, balance, and functional limitations
- Develop and implement individualized treatment plans
- Educate patients and caregivers on home exercise programs and safe mobility strategies
- Monitor progress, document outcomes, and adjust care plans as needed
- Collaborate with an interdisciplinary care team to ensure excellent patient outcomes
What You'll Need
- Active Florida Physical Therapist license
- Degree from an accredited Physical Therapy program
- Reliable transportation and valid driver's license
- Current CPR certification
- Strong communication and time-management skills
- Home health experience, preferred
