Job Description
Job Title: Social Worker - Pediatric Renal Service
Social Work Services at the Mount Sinai Hospital
The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.
The social worker in this position will be responsible for providing comprehensive and continuous social work services to children with renal disease and to their family members in collaboration with the current part-time pediatric renal social worker. Patients are followed in ambulatory care settings and in various in-patient pediatric settings, including the PICU and the dialysis unit. The social worker assists families with the impact of the diagnosis of a renal disorder or end stage renal disease and its implications throughout the course of the illness and through varied treatments including dialysis and transplantation.

LCSW preferred.
Skills in psychosocial assessment and knowledge of child/adolescent development.
Strong organization and writing skills.
Pediatric transplant experience preferred.

Responsibilities
The social worker functions as a member of an interdisciplinary team of physicians, nurses, a child life specialist and others and participates in a range of team-based activities including quality assurance and the development of mandated short- and long-term care plans. The worker must be able to deliver individual, family and group level interventions with the goal of facilitating the child and family’s adherence to care. Of critical importance is the ability to collaborate with multiple hospital and community-based resources and programs in order to enable a child to fully participate in treatment. In addition, this program is highly regulated; therefore, applicants must have strong organizational and writing skills to comply with regulatory requirements for documentation including initial psychosocial evaluations, transplant evaluation as mandated by federal standards, quarterly assessments, and short- and long-term treatment planning.
