Residential Case Manager - Trauma-Informed Care & Life Skills

Axiom Care in Phoenix, AZ is seeking a Case Manager to coordinate and ensure continuity of care under supervision of the Clinical Director. You will provide life skills training, connect clients to community resources, and facilitate groups within residential treatment. Proficiency with EMR and Microsoft Office, strong communication, and a trauma-informed approach are essential.
