PrimeCareHealth Community Health Centers is seeking a Behavioral Health Provider to deliver substance use and co-occurring disorder treatment within a team-based care model integrated with primary care. The role focuses on MAT coordination and collaboration with Primary Care Providers and SUD Care Coordinators across PrimeCare.
