About the position: The Community Health Nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information, education, referral and evaluation. Creates a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community. What You'll Do: 1. Direct Patient Care:
- Comprehensive Assessments: Evaluate patient health, identify needs, and develop care plans.
- Intervention and Management: Provide ongoing care for chronic conditions like depression, diabetes, asthma, etc.
- Medication and Treatment: Administer medications and treatments as needed.
- Urgent Care: Respond to urgent medical needs both in-person and remotely.
- Health Education: Empower patients to manage their health through education.
- Care Coordination: Work closely with the healthcare team (doctors, nurses, etc.) to ensure coordinated and effective care.
- Referrals: Manage referrals to specialists and other healthcare providers.
- Team Support: Act as a resource for other team members, providing expertise and guidance.
- Clinical Guidance: Provide mentorship and coaching to other team members.
- Delegation: Assign patient care tasks to team members based on their skills and scope of practice.
- Community Outreach: Build relationships with community partners to enhance patient care.
- Documentation: Maintain accurate patient records and documentation.
- Quality Improvement: Actively participate in quality assurance initiatives.
- Safety: Adhere to safe work practices and contribute to a safe work environment.

- An Associate's degree or a Bachelor's degree in nursing from an accredited nursing program
- Registered Nurse Licensure
- BLS CPR Certification
