JOB DESCRIPTION- About Facility:
- Home Health
- Radius rule: Traveler's permanent address must be 50 miles outside of home unit
- Department Information:
- 6 visits a day
- There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
- Minimum Years of Experience required? 1 year Home Health
- Will you accept a first-time traveler? Yes, if seasoned to home health
- Patient Types/Common Diagnoses:
- All Adults
- Chronic diseases, Orthopedics/Ortho Surgery
- Teammates will travel throughout the below areas: Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy and cities in between
- Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
- Driver's License and proof of Insurance required
- PT: BLS; State Licensure required
- OT: BLS; State Licensure required

- 5 days orientation
- Scheduling:
- Weekend rotation none
- No call
- No major holidays
- RTO: Manager approval
- Schedule cycle: every 13 weeks

- Additional Information:
- Do they schedule their own appointments with this position? - They will schedule the visit with the patients the day prior to the visit planned.
- Do they still get paid for cancelled appointments? Cancelled appts are not counted as visit equivalents. They are required 30 VE a week
- Will a company car be available? No
- Is a company cell phone provided? A phone is provided and is used for documentation and calling patients/providers.
