Conduct thorough evaluations of patients in both inpatient and outpatient settings while crafting customized treatment strategies

Engage collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to achieve optimal patient outcomes

Provide education to patients and families regarding therapeutic objectives, discharge planning, and recommended home exercises

Document assessments, treatment progress, and results in compliance with facility protocols and regulatory standards

Participate actively in care team discussions and interdisciplinary meetings to promote coordinated and holistic care

Doctorate in Physical Therapy (DPT) or a similarly accredited degree

Active state licensure as a Physical Therapist

CPR certification is preferred

Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Prior experience is highly desirable

Adeptness in treating patients with intricate medical and mobility challenges

Excellent communication and teamwork abilities

Proficient in using electronic health records and clinical documentation tools
