As a Home Health Caregiver, you will be responsible for providing direct patient care, maintaining a clean and safe environment, and collaborating with our multidisciplinary team to ensure the best possible outcomes.Responsibilities:Provide direct patient care, including assistance with activities of daily living (ADLs), such as bathing, dressing, grooming, and toiletingMaintain accurate and detailed records of patient care and progressImplement and update patient care plans in collaboration with the healthcare teamAssist with medication administration, wound care, and other medical treatments as neededMaintain a clean and safe environment, including laundry, housekeeping, and meal preparationCommunicate effectively with patients, families, and healthcare team members to ensure seamless care coordinationCollaborate with our multidisciplinary team to develop and implement individualized care plansParticipate in continuous quality improvement initiatives to enhance patient care and outcomesRequirements:High school diploma or equivalent requiredCertification in home health care or equivalent experienceAbility to lift, transfer, and reposition patients, as neededEffective communication and interpersonal skillsAbility to work independently and as part of a teamWork Environment:Work in clients' homes in the Gretna, Louisiana areaMust be able to travel to and from client homes, as neededAbility to work varied shifts, including days, evenings, and weekendsBrightCare HomeCare Offers:Competitive compensation packageOpportunities for professional growth and developmentCollaborative and supportive work environment
