Physical Therapy Assistant (PTA)
Flagstar Rehab is actively seeking passionate Physical Therapy Assistants (PTAs) to join our growing team in Skilled Nursing Facilities. If you're looking for a role that offers flexibility, consistency, and a strong support system, we're here to help you thrive.

For those pursuing independent contracting, we also provide guidance and LLC support.
At Flagstar Rehab, you can expect a smooth experience from start to finish, clear communication, quick placement, and a team that's always accessible when you need support.
Hourly Compensation:
- W2: $34$36
- 1099: $38$40
Physical Therapy Assistant Requirements and Responsibilities:
- Current NYS PTA License and Registration
- Valid BLS Certification
- EMR Competence
- Meets all State and Federal requirements for US employment in a Nursing Facility
- Malpractice/Liability Insurance (if applicable)
- Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
- Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
- Monitor and document patient progress
