Home Health RN Case Manager
Job Type: Full-Time Preferred | Part-Time Available Location: North Shore & Northern Chicago, Illinois Facility Setting: Home Health Patient Population: Primarily Geriatric Patients
Monday-Friday during regular business hours No on-call requirements No required weekends Optional weekend visits available
We are seeking compassionate Registered Nurses to provide skilled home health services, coordinate patient care, and manage clinical documentation while serving patients within designated geographic territories.
Provide skilled nursing care in patients' homes Perform assessments, medication management, and wound care Coordinate care with physicians and interdisciplinary teams Educate patients and families on treatment plans and health management Maintain accurate clinical documentation and regulatory compliance
Active Illinois RN License in good standing Current CPR Certification Skilled Home Health experience required Experience with Axxess EMR preferred Reliable transportation required Strong communication and patient care skills
Minimum 1-2 years of Home Health nursing experience required Skilled Home Health experience required
Per-visit compensation model Higher rates for Start of Care (SOC) visits Compensation varies based on visit type and experience
