We are hiring skilled and compassionate Registered Nurses for the Care Team Lead role at 2nd Family Home Care and Support services! [Primarily Remote]

We are currently hiring Caregivers for the greater Seattle area and Eastside. Candidates are selected for interviews based on their qualifications and will have an opportunity to communicate preferred case load, geography, and availability during their interview.

Job Description
This role is critical to our mission and sets the 2nd Family experience apart. Care Team Leads apply strong communication, clinical experience, problem-solving, and coaching skills, to keep the relationship between Caregivers (NAC), clients, and their loved ones running smoothly and ensure we deliver with excellence.
As a Care Team Lead, you will:
- Conduct initial consultations, including assessments, and document care plans
- Re-assess clients every 90 days (or with change of condition) and update care plans
- Orient assigned Caregivers (NACs) to care plans and assess their skills
- Act as a primary point of contact for clients and their loved ones, when questions or concerns arise
- Communicate with and support Caregivers (NAC), providing direction and mentorship as needed
- Attend a monthly company meeting (virtually or in-person)
- Occasionally support New Employee Orientation and in-service training
- Contribute to the overall success of the company by providing input and expertise
Schedule
The Care Team Lead role is primarily remote and responsibilities are addressed as needed, or on a self-directed basis. Exceptions include conducting in-person assessments to build out your client base, and participating in occasional training or team meetings.
General guidelines and expectations:
- Review Caregiver notes for each shift and follow up as needed
- Be available to take calls or respond to emails during on-duty hours (7 am to 7 pm, unless otherwise agreed upon)
- Respond to occasional emergency communication during off-duty hours
- Be able to conduct the initial consultations or change of condition assessments within 24-48 hours from request
- Schedule 90-day reassessments within the acceptable window
Compensation
Compensation is based on the number of assessments conducted and the number of active clients in the case load. Case load maximums will be determined in partnership between the Care Team Lead and Operations Director.
- Initial Assessment: $150 + mileage
- 90-reassessments: $75 + mileage
- All other client management: $50/client/week
- Orientation, in-service, or other support: Fees vary
