RN – Case ManagementSeven Healthcare are seeking an RN Case Manager specializing in case management for a travel contract role in the Colorado Springs/Woodland Park area. Enjoy the following benefits:License reimbursement – financial support to maintain your professional credentialsReferral program – earn rewards for introducing fellow healthcare professionalsHealth insurance – comprehensive medical coverage for you and your familyCertification reimbursement – assistance with obtaining additional qualificationsWeekly pay – prompt and reliable payments every weekKey Responsibilities of the RN Case Manager:Perform PRN/focused and routine assessments of high risk membersCollaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the memberAddress, identify, and continuously reassess cost-efficient, appropriate levels of carePut non-medical support in place to ensure compliance with treatment plans (such as housing and transportation)Coordinate care transitions as neededCoordinate with physicians, social workers, discharge planners, and claims professionals in transitioning patients to appropriate levels of careEngage specialty resources as needed to achieve optimal resolutionKeep detailed records of clinical, functional, and fiscal outcomes during the management processIdentify opportunities for health promotion and illness preventionPrevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomesAct as a patient advocate by protecting privacy and confidentiality issuesRequirements of the RN Case Manager:CO RN or Multi-State LicenseAssociate or bachelor's degree in nursing or related fieldCurrent, unrestricted state Registered Nurse licenseAbility to analyze complex medical information and make rational decisionsCase Management certificationReliable transportation and car insurance requiredHCHB Experience – Hospice CareColorado Springs: Where the Job Meets LifestyleEmbrace the vibrant culture and natural beauty of Colorado Springs.
