Job Summary

Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Manage and coordinate patient care across multiple healthcare settings
• Develop and implement individualized care plans and discharge plans
• Ensure compliance with health care regulations and standards
• Collaborate with healthcare professionals to enhance patient outcomes
• Utilize EMR systems for effective documentation and communication
• Oversee utilization management to optimize resource allocation

Qualifications

• 2 years of experience in a hospital setting required
• Active Compact RN License at the time of submission
• BLS Certification required
• NH RN License required at the time of submission
• Acute Hospital RN Case Management experience
• Critical care experience
