- Provide comprehensive care to patients in the skilled nursing facility (SNF), including assessment, diagnosis, and treatment planning.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients, families, and staff about medical conditions, treatment options, and preventive care.
- Perform routine medical procedures, such as wound care and diagnostic testing, within the scope of practice.
- Ensure compliance with all regulatory and facility standards of care.
- Act as a liaison between patients, families, and healthcare providers to facilitate optimal outcomes.
- Current and unrestricted Nurse Practitioner license in Kentucky.
- Master's or Doctoral degree in Nursing from an accredited program.
- Certification as an NP (Adult, Family, or Geriatric focus preferred).
- Minimum of 1-2 years of experience in a skilled nursing or long-term care setting.
- BLS certification is required; ACLS is preferred.
