Procedures: PT Rehab unit procedures follow a structured, evidence-based process that includes patient screening, comprehensive functional evaluation, and development of an individualized plan of care. Physical Therapists perform therapeutic exercises, manual therapy, gait and balance training, use of modalities, assistive device training, and wound care as indicated. Patient progress is continuously assessed, with treatments modified based on response and functional outcomes. Therapists collaborate with the interdisciplinary care team, provide patient and caregiver education, supervise support staff, and maintain accurate clinical documentation. Procedures also include discharge planning, outcome evaluation, and strict adherence to regulatory, safety, and professional practice standards to promote safe and effective rehabilitation.

Unit Description: Physical Therapy Rehab units provide comprehensive evaluation and rehabilitation for patients with physical impairments and wound care needs. Physical Therapists use standardized assessments and evidence-based practices to develop individualized plans of care, deliver therapeutic interventions (exercise, manual therapy, gait training, modalities, assistive devices, and wound care), and monitor patient progress. Therapists adjust treatments based on outcomes, coordinate care with the interdisciplinary team, educate patients and caregivers, and ensure accurate documentation, regulatory compliance, and discharge planning. The unit supports professional collaboration, staff supervision, continuing education, and adherence to APTA and state practice standards to optimize patient recovery and functional independence.
