Description Opportunity Information Sutter Medical Group is seeking a Post‑Acute & Geriatric Medicine Physician to join our physician‑led post‑acute care team in Sacramento, CA. You will serve as the primary physician of record for patients in skilled nursing and rehabilitation settings, managing complex medical conditions, leading interdisciplinary care, and helping patients recover, stabilize, and avoid unnecessary hospital readmissions. Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred). Active and unrestricted medical license in California. Demonstrated experience in post-acute, skilled nursing, long‑term care, or geriatric settings. Strong clinical skills in managing complex, multi-morbid patients. Excellent communication and interpersonal skills, with a patient- and team-centered approach. Ability to work effectively in an interdisciplinary, facility-based environment. Proficiency in electronic medical records (EMR) systems. Preferred Qualifications Fellowship training in Geriatric Medicine or experience in post‑acute/rehabilitation medicine. Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics. Experience participating in value‑based care or population health programs.
