Experience / Qualifications Licensed Physical Therapist. Position open to PTs of all experience levels. Key Responsibilities Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care following all regulatory and clinical practice standards. Utilize standardized assessments and evidence‑based practice to support clinical interventions. Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements. Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices. Assess patient’s progress and adjust treatments accordingly in the clinical record. Evaluate treatment outcomes for effectiveness and modify services or provide referrals as needed. Provide consultation and counseling to patients, families, caregivers, and other service providers related to the physical disorders. Communicate effectively with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
