LPN Case Manager

The Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) Case Manager supports the Transitions of Care program focused on reducing 30-day hospital readmission rates. Working under the direction of an RN Care Manager, the LPN assists with implementing care plans through appointment scheduling, biometric data monitoring, patient education, home and office visits, and coordination with community resources. This mobile role requires strong communication, documentation skills, and the ability to work independently as part of a multidisciplinary team managing a caseload of 25 30 patients. Responsibilities:

• Assist RN Care Manager in implementing patient care plans.
• Schedule patient appointments and monitor biometric data.
• Provide and reinforce patient education regarding treatment and medication adherence.
• Conduct home visits and attend patient visits as needed to observe and educate.
• Maintain frequent contact with patients and connect them with community resources.
• Document patient interactions and care activities accurately in the electronic health record (EHR).
• Participate in multidisciplinary care team rounds to provide patient updates.
• Manage a monthly rolling caseload of approximately 25 30 patients.
• Support care coordination efforts to reduce hospital readmissions.
• Work collaboratively with social workers, pharmacists, and other care team members.

• Current LPN/LVN licensure in the state of practice.
• Basic Life Support (BLS) certification.
• Minimum of 1 year of experience in home health, case management, or related nursing field.
• Strong clinical knowledge, preferably including CHF, COPD, and complex comorbidities.
• Excellent written and verbal communication skills.
• Proficiency in electronic health record documentation.
• Reliable transportation for travel to patient homes, hospitals, and provider offices.

• Experience with care coordination and community resource linkage.
• Prior experience in transitions of care or case management programs.
• Ability to work independently and adapt to a flexible schedule.

• This is a mobile position requiring travel to patient homes, hospitals, and provider offices.
• Role is non-hands-on nursing; direct clinical procedures are not performed.
• Participation in virtual care team rounds is required.
• Work involves telephonic follow-up and patient triage.
• Must maintain clear, concise communication and documentation.

• Full-time position with a guaranteed 40-hour workweek.
• Typical schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m.
• No weekend or Saturday work required.
• Flexible scheduling may be necessary to accommodate patient visits and care team activities.
