Overview Certified Occupational Therapy Assistant (COTA) - Home Visits Staten Island, NY Join a growing home care team where compassionate care, flexibility, and clinician support come first. This is a full-time position. In order to be considered for this job you must have a valid COTA license in the state of NY. Pay: $80,000.00 – $85,000 per year Job Details Licensed/regulated as a Certified Occupational Therapy Assistant (COTA) in the practicing state, if required. Responsibilities Assess patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) and assess home environment for hazards or barriers to more independent living. Identify equipment needs. Participate in the development of the total plan of care. Execute treatment program and goals for improved function. Communicate plan of care to referring physician and other organization professionals. Teach new skills or retrain patient in daily activities that have been lost due to illness or injury. Fabricate splints and instruct patient in the use of adaptive equipment to improve function. Train patient in the use of prosthetic and/or orthotic devices. Maintain clinical records, clinical notes, summary reports and reports to the physician of any changes in the patient’s condition. Submit these records in accordance with policy. Maintain contact with other personnel involved in the patient’s care to promote coordinated, efficient care. Attend and contribute to in-services, case conferences and other meetings as required to ensure coordinated and comprehensive plans of care. Identify patient and family/caregiver needs for other home health services and refer as appropriate. Instruct patient’s family/caregiver and other health care personnel in the patient’s treatment regime when indicated and appropriate. Participate in discharge planning for the patient. Graduate of an ACOTE-accredited Occupational Therapy Assistant program. Strong verbal, written, and organizational skills. Pediatric experience or willingness to work with pediatric patients required.
