Position Summary

Baywood Post-Acute is seeking a skilled and compassionate Speech Therapist (SLP) to provide high-quality speech, language, cognitive, and swallowing services to our residents. The Speech Therapist will evaluate residents, develop individualized treatment plans, provide skilled therapy interventions, and monitor progress while collaborating with an interdisciplinary team.

The ideal candidate is passionate about patient-centered care and enjoys working in a collaborative Skilled Nursing Facility environment.

Essential Duties and Responsibilities

• Conduct comprehensive evaluations of residents with speech, language, cognitive, communication, and swallowing disorders.

• Develop individualized treatment plans based on evaluation findings, physician orders, resident needs, and established goals.

• Provide skilled speech-language pathology services in accordance with the plan of care.

• Provide assessment and treatment for dysphagia and other swallowing-related disorders.

• Address speech, language, communication, and cognitive deficits through evidence-based therapeutic interventions.

• Monitor resident progress and modify treatment plans as clinically appropriate.

• Maintain accurate, timely, and complete clinical documentation.

• Participate in care plan meetings and collaborate with the interdisciplinary team.

• Communicate effectively with nursing, physicians, dietary staff, rehabilitation professionals, residents, families, and caregivers.

• Educate residents, families, and caregivers regarding treatment plans, therapeutic strategies, precautions, and recommendations.

• Make appropriate recommendations regarding diet consistency, swallowing precautions, communication strategies, and adaptive techniques within the scope of practice.

• Maintain compliance with facility policies, professional standards, and applicable federal and state regulations.

• Promote resident safety, dignity, independence, and quality of life.

• Perform other duties as assigned.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited institution.

• Current and unrestricted California license as a Speech-Language Pathologist.

• ASHA Certificate of Clinical Competence (CCC-SLP), as applicable.

• Previous experience in Skilled Nursing, post-acute care, rehabilitation, or geriatric settings preferred.

• Strong clinical assessment, treatment planning, documentation, and communication skills.

• Ability to work effectively and professionally as part of an interdisciplinary team.

• Commitment to providing compassionate, resident-centered care.
