Responsibilities Establishes and maintains a therapeutic relationship with the patient and family, encompassing an understanding and integration of cultural and diversity into practice Communicates and works collaboratively with the patient, family, and health care team members Respects the patient's values, preferences, expressed needs and knowledge of the healthcare situation in holistic data collection, in formulating health care outcomes and in the evaluation process Utilizes the ANA Scope and Standards of Practice (2015) nursing process components: assessment, diagnosis, outcomes identification, planning, implementation and evaluation Assessment: collects pertinent data and information relative to the healthcare consumer's health or situation Diagnosis: analyzes the assessment data to determine the actual and potential diagnoses or the issues Outcomes Identification: identifies expected outcomes for a plan individualized to the healthcare consumer or the situation Planning: develops and modifies a plan of care that prescribes strategies and alternatives to attain expected outcomes Implementation: implements the identified plan Coordination of Care: coordinates care delivery and transitions in care Health Teaching and Health Promotion: employs strategies to promote health and a safe environment Evaluation: evaluates progress toward attainment of outcomes Ethical practice in all areas Practices in a culturally congruent manner Communicates effectively in all areas of practice Collaborates with healthcare consumers and other stakeholders in nursing practice Leads within the professional practice setting and the profession Seeks knowledge and competence reflecting current nursing practice and promoting futuristic thinking Integrates evidence and research findings into practice Contributes to quality nursing practice Evaluates own and others' nursing practice Utilizes resources appropriately to plan, provide, and sustain evidence‑based nursing services that are safe, effective, and fiscally responsible Practices in an environmentally safe and healthy manner Qualifications Graduation from an accredited school of nursing Currently licensed and registered as a Registered Nurse in the State of Connecticut Unit Context HVC Cardiac Medical Intermediate Care Unit (5-2 SP): 26‑bed cardiovascular medicine inpatient unit with medical and interventional management of coronary artery disease and atrial/ventricular dysrhythmias. HVC Cardiac Surgery Intermediate Care and Step Down Unit (5-3 SP): 25‑bed cardiac surgery unit encouraging post‑operative management for CABG, valve replacement and aneurysm repairs. HVC Cardiac Intermediate Care and Stepdown Unit (V3E): 30‑bed unit at Saint Raphael Campus, primary population includes patients with cardiac procedures. V5E – Advanced Heart Failure and Stroke Unit: 29‑bed unit at Saint Raphael Campus handling advanced heart failure, MI, catheterization, pacer/ICD placement, stroke patients post TNK, etc. HVC Cardiac Medical Intermediate Care Unit (5-7 EP): 26‑bed telemetry heart and vascular unit, not a med‑surg unit, for general cardiology, post‑cardiac procedure, heart failure, vascular surgery patients.
