Job Summary:
A Long Term Care Registered Nurse (LTC RN) position is available in Berlin, Wisconsin. This role involves providing patient care in a long-term care and rehabilitation setting, with a focus on wound care, following physician orders, and PIV insertion/blood draws. Candidates are required to bring their license on the first day of employment.

Job Details:

• Location: Berlin, Wisconsin
• Certifications: Wisconsin Registered Nurse (RN), American Heart Association (AHA) Basic Life Support (BLS) certification
• Education: Associates Degree or Certificate
• Experience: Minimum of 2 years of recent long-term care experience required; experience with wound care, PIV insertion, and blood draws preferred; Epic charting system and Pyxis medication system highly preferred
• Schedule: 12-hour days, five weeks contract, starting 06/29/2026, with a guaranteed 36 hours per week
• Required Qualifications: Wisconsin RN license, Associates Degree or certificate, recent LTC experience with wound care and follow-up of medical orders, PIV and blood draw experience, Epic preferred, Pyxis highly preferred, and at least 2 years of experience
• Additional Information: Ability to float to Peabody Manor (40 miles), work every other weekend, rotate holidays as needed, and complete a 5-day orientation (including 1 day onboarding and 4 shifts with a preceptor). No on-call or callback requirements. Billing occurs from Sunday to Saturday with unbillable orientation hours. Set hours guaranteed per week, except during holidays when the unit is closed. The start date is ASAP.
