Job Summary

Join a dedicated healthcare team at a Skilled Nursing Facility (SNF) in Alturas, California. This role is integral to providing comprehensive, individualized care for residents within a supportive environment.

The Certified Nursing Assistant (CNA) collaborates closely with nursing staff, residents, families, and various facility departments to ensure optimal resident well-being. Responsibilities include direct patient care, monitoring resident conditions, and maintaining clear communication with the interdisciplinary team.

Key Responsibilities

• Receive and provide thorough reports on resident conditions and care plans.
• Conduct regular rounds, perform initial assessments for new residents, and evaluate residents experiencing changes in condition, falls, or injuries.
• Accurately document resident assessments and care activities in compliance with facility policies.
• Promptly communicate any changes in resident status or immediate needs to the nursing staff.
• Assist residents with activities of daily living (ADLs) and ensure timely response to call lights.
• Cultivate positive and collaborative relationships with co-workers, residents, families, and physicians.
• Perform other duties as assigned to support resident care and facility operations.

Required Qualifications

• Active Certified Nursing Assistant (CNA) license in California.
• Demonstrated ability to work effectively as part of a healthcare team.
• Excellent verbal and written communication skills.
