Job Title: Community Engagement Coordinator Dept: Prevention & Harm Reduction Services Reports to: Harm Reduction Manager Salary Range: $25.00 – $28.00 hourly Position Summary:The Community Engagement Coordinator supervises a harm reduction team serving BIPOC and justice-involved people who use drugs (PWUD), overseeing outreach, client engagement, staff supervision (Prevention Care Managers and Engagement Specialists), quality assurance, and care coordination with external partners. The role ensures services align with harm reduction, trauma-informed, and culturally responsive principles while maintaining compliance with grants and agency standards. Responsibilities include staff training, advocacy, programmatic reporting, and building community linkages to reduce barriers to care. Authority and Independence The Community Engagement Coordinator works under general supervision and exercises independent judgment within established policies, procedures, and grant requirements. The position requires maintaining confidentiality, balancing supervisory responsibilities with direct program oversight, and managing outreach, engagement, and coordination activities with professionalism and cultural humility. Minimum Qualifications Education and Experience Bachelor’s degree; OR a minimum of 2+ years’ experience working in a non-profit social service agency providing direct client services to diverse populations (e.g., PWUD, justice-involved individuals, people with mental health diagnoses), or an equivalent combination of education and experience. Case management and/or healthcare experience. Experience working within harm reduction, trauma-informed, or culturally responsive frameworks preferred. Experience with Electronic Health Record (EHR) systems preferred. Skills and Competencies Excellent organizational and time management skills. Strong written, verbal, and documentation skills. Computer proficiency, including Microsoft Office; experience with EHR/data systems preferred. Sensitivity to HIV/AIDS, chronic illness, substance use, and a strong commitment to ACR Health’s mission, vision, and values. Ability to work respectfully and effectively with people who are BIPOC, LGBTQI+, justice-involved, experiencing homelessness, or actively using substances. Fluent in both Spanish and English preferred. Lived experience of homelessness, substance use, BIPOC identity, and/or LGBTQI+ identity strongly preferred. Other Requirements Valid New York State driver’s license and reliable access to a vehicle with current insurance for work-related travel, training, outreach, and occasional client transport (personal vehicle ownership is not required but consistent access is necessary). Successful completion of a background screening and verification process, including checks required under state Medicaid/OMIG guidelines. Primary Responsibilities Maintain strict confidentiality in compliance with Article 27-F, HIPAA, and ACR Health policies; protect agency data in all job duties and communications. Oversee outreach and client recruitment efforts to ensure consistent, effective engagement of priority populations, particularly BIPOC and justice-involved PWUD. Supervise Prevention Care Managers and Engagement Specialists, providing regular support, coaching, performance feedback, and case-specific consultation for complex client situations. Ensure all services are delivered in alignment with harm reduction principles, trauma-informed care, and culturally responsive practices. Conduct routine quality assurance (QA) reviews and lead quality improvement (QI) initiatives to strengthen service delivery and data integrity. Review and submit timely programmatic reports; track performance metrics and ensure compliance with grant and agency requirements. Monitor outreach, engagement, and care coordination activities to ensure program fidelity and responsiveness to community needs. Coordinate with external partners (e.g., culturally comprehensive providers, housing and medical services, justice system entities) to support referrals, warm handoffs, and collaboration. Facilitate internal team meetings and case conferencing to promote cross-collaboration, shared decision-making, and consistent communication. Support onboarding, training, and professional development of staff, particularly team members with lived experience. Attend meetings, trainings, and calls designated by the New York State Department of Health (NYSDOH); represent the program in alignment with its goals and values. Ensure proactive and productive links with area social service providers and community-based organizations. Authorize and arrange for necessary support, medical, and stabilizing services to help clients develop and maintain effective health care and stability. Reduce barriers to services through persistent advocacy and appropriate documentation of service gaps. Advocate for thorough and responsive service acquisition and systems improvements, including representing clients when advocacy is needed. Maintain current knowledge of local service providers and client resources; stay informed about developments in medical, behavioral health, social services, legal, and related fields. Complete all required staff, department, and ACR Health training, including a minimum of 24 hours annually of professional development and certifications. Provide crisis intervention as appropriate and within scope. Maintain working knowledge of all applicable program regulations, policies, procedures, and required database/EMR documentation standards. Attend all required and recommended staff trainings, supervisions, and in-services. Perform other duties as directed by the Health Homes Manager, Director of Care Management Services, Chief Program Officer, and/or Chief Executive Officer.
