• Phone Consults: Yes • Case Mix: General anesthesia, GI procedures, OB, Cath Lab interventions FACILITY INFORMATION • No. of Beds in Facility: 268• Office Equipment Available: 11 surgical suites (including Hybrid OR), 6 outpatient suites, GI labs, OB department, Cath Labs • Specialty Backup Available: Yes – 145 physicians representing 32 specialties • Support Staff Available: CRNAs under anesthesiologist supervision • Reason for Coverage: Expansion PRIVILEGES & COMPLIANCE • Hospital Privileges Required: Yes
