Park Gardens Rehabilitation & Nursing Center , a skilled nursing facility, located in Bronx County, New York , is actively looking to hire a clinically superb  full-time LPN/RN Charge Nurse. The LPN/RN Charge Nurse will manage an assigned caseload to assure the quality of services provided meet the high standards of professionalism and those required by City, State and Federal Regulations.

Job Duties

• Oversee and coordinate nursing care for assigned patients.

• Guide and support field nurses; serve as a clinical resource.

• Review clinical documentation for accuracy, completeness, and compliance.

• Communicate with patients/families, physicians, insurance case managers, and internal teams to advance safe, timely care.

• Participate in quality improvement and adhere to agency policies/standards.

Requirements

• Valid NY LPN/RN license

• Associate’s degree required; BSN preferred.

• 2+ years nursing home experience (minimum).

• Strong communication and computer skills

  Positions Available : Fulltime, Parttime and Per Diem positions available.
