This role conducts clinical assessments, develops and updates care plans, collaborates with physicians and the interdisciplinary team, and ensures safe, timely transitions of care. The RN Case Manager educates patients and families, facilitates authorizations, monitors quality and length of stay, and supports regulatory and documentation standards, contributing to LifeBridge’s mission of compassionate, community-focused, innovative care.
