Home Care Coordinator

InterimHealthCare is seeking an organized, compassionate, and dependable Care Coordinator to ensure our clients receive consistent, high-quality care. Key Responsibilities Manage daily schedules and ensure all client shifts are properly staffed. Confirm caregiver attendance and review daily care notes. Respond promptly to scheduling conflicts and service concerns. Speak with incoming leads and explain available home care services. Complete initial in-home assessments and client visits. Coordinate first shifts and follow up with clients and families. Maintain accurate client documentation in AxisCare. Communicate closely with caregivers, supervisors, clients, and families. Build relationships with prospective clients and referral sources. Ensure compliance with care plans, agency policies, and state requirements. The ideal candidate is a strong communicator and problem-solver who can manage multiple priorities in a fast-paced environment. Home care, healthcare scheduling, client services, or care coordination experience is preferred.
