Home Health Physical Therapist

Seeking a Home Health Physical Therapist. The role involves providing therapy services to patients in their homes. The position requires performing admission evaluations, reassessments, and discharge planning. The therapist will primarily work with patients experiencing general weakness, back pain, and orthopedic issues. The caseload covers multiple counties, with adjustments made based on travel time and mileage. The therapist will be responsible for ensuring the quality of care and adhering to safety protocols. This position requires effective communication and interpersonal skills to educate patients and families. Traveling to various locations is a key aspect of the role. Participation in interdisciplinary support with other healthcare professionals is expected. The ideal candidate will be adaptable to changing patient needs and demonstrate strong time management skills. Important skills include the ability to develop and implement individualized care plans. The candidate will also be involved in promoting patient independence through therapeutic exercises and interventions. Experience with electronic charting systems, preferably Epic, is preferred. Previous travel experience is beneficial.
