Flexible part-time or per diem scheduling Choose block scheduling or guaranteed daily hours Competitive pay Mileage reimbursement Independent field-based role with strong office support One-on-one client visits Paid documentation time built into your schedule Opportunity to educate and mentor caregivers Family-owned company with a supportive culture Help seniors remain safe and independent in their own homes Flexible Scheduling Options Block Schedule Choose the days and hours you're available Paid for your scheduled block of time Our office coordinates client visits, travel time, and documentation Guaranteed Hours Guaranteed 5-8-hour workdays available Collaborative scheduling with office staff Typical expectation of approximately four client visits during a five-hour shift, depending on travel distance What You’ll Do Complete comprehensive in-home client assessments Develop, review, and update individualized care plans Conduct caregiver orientation and quarterly supervisory visits Educate, coach, and support caregivers to ensure quality care Complete documentation within required timeframes Monitor client progress and communicate changes to the care team Collaborate with office staff to ensure continuity of care Participate in caregiver education and in-service training Perform routine nursing visits as needed, including medication management, blood pressure checks, catheter changes, and other nursing services within your scope of practice Provide hands-on assistance during urgent situations when needed Aging Services Access Point (ASAP) Responsibilities Complete in-home assessments and develop Home Health Aide Plans of Care Match caregiver skills with client care needs Orient and supervise caregivers assigned to ASAP clients Complete quarterly supervisory visits and annual reassessments Collaborate with ASAP nursing staff regarding client status and care plan updates Identify changes in client condition and advocate for appropriate services Report required incidents and changes according to ASAP guidelines Qualifications Current Massachusetts Registered Nurse (RN) license Home care, community health, case management, or visiting nurse experience preferred Strong assessment and clinical judgment skills Excellent communication and documentation skills Ability to work independently Reliable transportation, valid driver's license, and automobile insurance Service Area Boston Brookline Newton Needham Wellesley Cambridge Somerville Medford Malden Chelsea Revere Everett Belmont Watertown And surrounding communities Travel is scheduled efficiently whenever possible to minimize drive time and maximize time spent with clients.
