About the Position

GiaCare Inc. is seeking a Licensed Clinical Social Worker (LCSW) to serve as a Social Worker Case Manager in support of the Mental Health Clinic at Womack Army Medical Center in Fort Bragg, North Carolina.

This position plays a critical role in coordinating behavioral healthcare for military service members and other eligible beneficiaries, including individuals transitioning from inpatient psychiatric care, emergency-department treatment, or hospitalization following a nonfatal suicide attempt.

The LCSW Case Manager will lead mental health case-management functions, coordinate care across multiple levels of treatment, support patients enrolled in the High Risk Suicide Pathway, and work closely with patients, families, commanders, clinical teams, and community providers.

Schedule

• Monday through Friday
• Eight-hour shifts totaling 40 hours per week
• Scheduled between 7:00 a.m. and 6:00 p.m., based on clinic requirements
• No weekends, federal holidays, on-call, or callback requirements

Responsibilities

• Serve as the Mental Health Case Management Function Leader and support the effective performance of case-management operations.
• Coordinate care and follow-up planning for patients transitioning from inpatient psychiatric care, emergency-department treatment, or medical hospitalization.
• Support patients enrolled in the High Risk Suicide Pathway through regular face-to-face contact, safety monitoring, and care coordination.
• Conduct suicide-risk assessments, develop and monitor collaborative safety plans, provide crisis intervention, and coordinate inpatient admissions when required.
• Review clinical documentation and collaborate with patients, families, healthcare providers, and multidisciplinary treatment teams to develop individualized care plans.
• Maintain consistent communication with high-risk patients until a complete handoff and connection to the next level of care has been established.
• Coordinate referrals and access to behavioral health, medical, transportation, housing, benefits, and community-support resources.
• Advocate for patients and families through military, community, school, court, and government resources when appropriate.
• Communicate with patient commanders in accordance with established clinical and military requirements.
• Coordinate and chair multidisciplinary treatment-team meetings.
• Participate in utilization-management and case-management meetings to identify opportunities for improved patient care.
• Collaborate with RN Case Managers in the development and implementation of the Case Management Program.
• Provide clinical supervision to unlicensed care providers and mental-health technicians.
• Provide counseling or psychotherapy services within the scope of professional licensure.
• Complete medical-record documentation, coding, tracking logs, and required data reporting.
• Participate in staff orientation, training, process-improvement initiatives, committees, work groups, and task forces.
• Provide education and support to patients and their families or caregivers to strengthen self-management skills and improve access to care.

Requirements

• Master of Social Work degree from a school fully accredited by the Council on Social Work Education.
• Current, unrestricted license to practice as a Licensed Clinical Social Worker in any U.S. state, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands.
• At least one year of experience within the past five years in the independent practice of clinical social work or healthcare management involving adults, children, families, seniors, or groups.
• Current Basic Life Support certification.
• Experience using an electronic health record system.
• Computer proficiency sufficient to complete clinical documentation, data reporting, and other required duties.
• Must be a U.S. citizen.
• Must be able to successfully complete all credentialing and security requirements.
