Job Description

As a General Outpatient Nurse within our Certified Community Behavioral Health Clinic (CCBHC), you will provide comprehensive nursing care that supports whole-person wellness and recovery. This role combines clinical expertise, care coordination, patient education, medication management, and regulatory compliance to ensure seamless, high-quality care delivery. Working as part of an interdisciplinary team, you will support patients across multiple levels of care, facilitate transitions following hospitalization, coordinate treatment services, and promote positive health outcomes through evidence-based nursing practices.
