Conducts bio-psychosocial assessments, provides intervention and treatment as indicated. Identifies high‑risk psychosocial factors of patients/families that impact health status and helps staff understand the influence of those factors upon the course of medical care. Required to identify and intervene for suspected abuse including sensitivity to age specific concerns: physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; caretaker/institutional abuse for senior adults. Must be able to manage crisis situations, demonstrating ability to quickly evaluate crisis situation and select appropriate intervention. Must be able to document direct service in patient’s medical record after each significant encounter. Must demonstrate proficiency in use of the electronic medical record. Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement. Required Work Experience: One (1) year of social work experience in a health care setting.
