WE’RE HIRING: MDS COORDINATOR
John Knox Manor Nursing Home

Are you an experienced MDS Coordinator who takes pride in accuracy, compliance, and quality resident care?
We’re looking for a knowledgeable, organized, and compassionate MDS Coordinator to join our team and play a key role in ensuring our residents receive the care they deserve.

WHAT YOU’LL DO

Coordinate and oversee the MDS assessment process
Ensure assessments are accurate, timely, and compliant with current CMS requirements
Collaborate closely with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary team members
Lead and facilitate care plan development and coordination
Monitor documentation and identify opportunities to improve clinical outcomes
Support regulatory compliance and survey readiness
Help ensure the interdisciplinary team is working together toward the best outcomes for our residents
Maintain certs/recerts
Initiate death certificates

WHAT WE’RE LOOKING FOR

Previous MDS Coordinator experience in a Skilled Nursing Facility
Strong knowledge of MDS 3.0 / RAI processes
Understanding of Medicare, Medicaid, and SNF reimbursement requirements
Excellent organization skills and attention to detail
Strong communication and leadership skills
A team player who is committed to resident-centered care
