Licensed Practical Nurse (LPN) – Skilled Nursing

Status: Full-Time / Part-Time / PRN
FLSA Status: Non-Exempt

Position Summary

The Licensed Practical Nurse (LPN) is responsible for providing quality nursing care to residents in accordance with physician orders, state and federal regulations, and facility policies and procedures. The LPN works closely with Registered Nurses, State Tested Nursing Assistants (STNAs), physicians, residents, and families to ensure the highest level of care while promoting dignity, respect, and resident-centered care.

Essential Job Duties

• Administer medications and treatments as ordered by the physician.
• Monitor residents' physical and emotional conditions and report any changes promptly.
• Complete accurate and timely nursing documentation.
• Assess residents and contribute to the development and implementation of care plans.
• Supervise and direct STNAs in the delivery of resident care.
• Respond to resident emergencies and initiate appropriate nursing interventions.
• Communicate effectively with physicians, residents, families, and the interdisciplinary team.
• Ensure infection prevention and control practices are followed.
• Maintain resident confidentiality in accordance with HIPAA regulations.
• Assist with admissions, discharges, and transfers.
• Complete physician orders accurately and promptly.
• Participate in shift reports and interdisciplinary meetings.
• Ensure compliance with all facility policies, Ohio Department of Health regulations, and federal guidelines.
• Promote a safe, clean, and positive environment for residents, visitors, and staff.
• Perform other duties as assigned.

Qualifications

• Current Ohio Licensed Practical Nurse (LPN) license in good standing.
• Current CPR certification (or ability to obtain upon hire).
• Previous long-term care or skilled nursing experience preferred but not required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Physical Requirements

• Ability to stand, walk, bend, lift, and move throughout the shift.
• Ability to lift, transfer, and reposition residents with assistance and mechanical lifting devices as needed.
• Ability to frequently lift up to 50 pounds.
• Ability to respond quickly in emergency situations.

Work Environment

• Skilled Nursing Facility
• Exposure to infectious diseases and bodily fluids.
• Requires use of appropriate personal protective equipment (PPE).
