Make a difference every day by connecting with families and helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT) to ensure safe, healthy, and independent lives. What You’ll Do Hit the road locally to visit clients in their homes. Serve as the go-to guide for families navigating waiver programs. Write up plans of care and assessments that make a difference. Troubleshoot challenges such as service authorizations, timesheets, and more. Build lasting relationships with families based on respect, patience, and trust. Work remotely from your laptop or tablet. What We’re Looking For At least two years of experience supporting individuals with disabilities or the elderly. No degree required; we value heart and experience. Tech‑savvy in email, documents, spreadsheets, portal navigation, and video calls. A valid driver’s license and a reliable vehicle. Excellent written and spoken communication skills. Ability to pass a background check and provide two professional references. Bonus: Person‑Centered Thinking/Planning training.
