Responsibilities
- Administer prescribed medications and treatments according to physician orders and facility protocol.
- Monitor residents' vital signs, assess health changes, and promptly report findings to the supervising RN.
- Provide direct patient care including assistance with activities of daily living, mobility, hygiene, and comfort measures.
- Document all care, observations, and interventions in the electronic health record; ensure accurate handoffs at shift change.
- Collaborate with RNs, physicians, therapists, and dietary/rehabilitation teams to implement individualized care plans.
- Maintain safety, infection control, and resident privacy; respond to emergencies and call lights as needed.

Requirements
- Current Alabama LPN license in good standing (or eligible for licensure)
- Graduation from an approved practical nursing program
- Minimum 1 year of licensed practical nursing experience in a long-term care or skilled nursing setting preferred
- Ability to work the 3x12-hour night shift schedule (18:00-06:00) with a 3-on/2-off and 2-on/3-off rotation
- Strong communication skills and ability to work effectively in a team-oriented, resident-centered environment
- Eligible for travel assignments and able to meet facility requirements for temporary placement
