About the Opportunity

The ideal NP will have 2–3+ years of primary care experience and be comfortable providing comprehensive care in a community-based setting.

You’ll work as part of a collaborative, multidisciplinary team providing accessible, respectful, and evidence-based primary care across a diverse patient population.

Schedule:

• 3 daytime shifts: 8:30 AM–4:30 PM
• 2 evening shifts: 12:00 PM–8:00 PM
• Optional Saturday: 9:00 AM–3:00 PM
• Some flexibility in scheduling, including the ability to have input into evening shifts

What You’ll Do

• Provide comprehensive primary care services across the lifespan
• Conduct evaluations, examinations, diagnoses, and treatment
• Manage follow-up care, test results, referrals, and ongoing patient needs
• Provide patient-centered care in a respectful and inclusive environment
• Document clinical care in EPIC
• Collaborate with physicians, nurses, behavioral health professionals, and other members of the care team
• Participate in quality improvement, case conferences, and professional development

Qualifications

• Current New York State NP license/registration
• Current DEA registration
• Current CPR certification
• 2–3+ years of primary care experience strongly preferred
• Community health experience a plus
• HIV experience strongly preferred
• Spanish/English bilingual skills are a plus
