Mainspring Recovery is a mission-driven behavioral health organization dedicated to helping individuals rebuild their lives. We seek a Case Manager to coordinate discharge planning, Medicaid enrollment, and follow-up scheduling across residential, PHP, and IOP services. You will collaborate with clinical and medical teams, arrange transportation, facilitate life skills groups, document in the EHR, and support timely, compliant transitions between levels of care. #J-18808-Ljbffr Mainspring Recovery
