Physical Therapist / Physical Therapist Assistant (PRN) – Continuing Care Retirement Community HTS is currently seeking dedicated and compassionate Physical Therapists and Physical Therapist Assistants to join our therapy team on a PRN basis at our partner continuing care retirement community located in Walnut Creek, Ohio (Holmes County). Key Responsibilities Provide high-quality physical therapy services to residents in a continuing care retirement community. Assist with managing increases in caseload, scheduled time off, and weekend needs on a PRN basis. Maintain accurate documentation of resident progress and treatment plans. Collaborate with the interdisciplinary team to develop and implement individualized care plans. Ensure compliance with all regulatory and safety standards. Adapt therapy sessions to meet the unique needs of each resident, promoting mobility, independence, and overall well-being. Skills and Qualifications Valid state licensure as a Physical Therapist or Physical Therapist Assistant. Prior experience in a long-term care or retirement community setting is preferred. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Flexibility to work weekdays and weekends as needed.
