Through the assimilation and application of evidence-based information, the NCM designs, implements, and evaluates client treatment plans by coordinating, delegating and participating in the care provided by the health care team. The NCM is responsible for working with the health care system to promote an environment that will empower his/her assigned patients to manage their illness(es) and achieve optimal outcomes. The NCM will provide and manage care of assigned community clients.Minimum RequirementsBSN or the equivalent in a combination of an ADN and relevant experienceCurrent Missouri License as a Registered NurseMinimum of three (3) years experience working in a healthcare settingExcellent written and verbal communication skillsMust be able to read and speak the English languageProficient in Word, Excel and Power PointCurrent BCLSDriving is a requirement for the employee.
