Travel Outpatient Physical Therapist - $2,448 per week

Certification Details BLS Massachusetts Physical Therapist license required. Job Details Outpatient only. 4-10 hour shifts. Shifts start between 8-8:30 am. Schedule Information 4 shifts per week. Each shift is 10 hours. Shift start times between 8:00 and 8:30 am. Job Requirements Massachusetts Physical Therapist license required. BLS certification required. Additional Details Shared housing provided at no cost. MGB Nantucket Housing Agreement required upon hire if staying in NCH housing. MGB FEMA ICE 100 Training must be completed, no exceptions. Documentation of TB test within 3 months of start date required. Health Statement signed by MD/DO/NP/PA within 365 days of start date required. MGB Criminal Background Check within 45 days prior to start date required. MGB Massachusetts CORI background check within 30 days prior to start date required. MGB Resume including past 7 years of work experience required for submission. 2 references within 4 years required, minimum 1 supervisor or above. MGB Skills Checklist must be completed within 1 year of submission. MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions. MGB Education Verification required. MGB SSN Trace required. MGB National Sex Offender Check required. MGB Exclusion Checks required.
