Job Description The Clinical Nurse Navigator uses their oncology‑specific clinical knowledge to offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, the Navigator provides education and resources to facilitate informed decision‑making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The role includes routine nursing aspects such as clinical care, telephone triage, symptom management, management of patients on oral chemotherapy, data gathering, chart updating, patient education, and monitoring and follow‑up of patients. The Navigator works collaboratively with schedulers, medical assistants, non‑clinical navigators, infusion nurses, and physicians in carrying out day‑to‑day clinical operations. Responsibilities Assume nursing care responsibilities for a defined group of patients within hematology/oncology, including assessment, education, coordination, and documentation. Act as the point of contact for physicians, patients, and caregivers to assist with coordination of care. Support smooth transition of patients from diagnosis and active treatment into survivorship, chronic cancer management, and end‑of‑life care. Serve as liaison with multidisciplinary team members and various departments to integrate pertinent patient information for seamless continuum of care. Receive and triage patient telephone calls and intervene appropriately, alerting physicians as needed. Follow up with patients by telephone to ensure compliance with treatment guidelines and evaluate changing or uncertain clinical situations. Work closely with Financial Advocates to facilitate financial support for all treatment modalities. Collaborate with Prior Authorization team to facilitate authorizations for all needed treatments. Provide psychosocial support and facilitate appropriate referrals for patients, families, and caregivers. Develop standardized care plans with physicians, including referrals, bone marrow transplants, drug protocols, and clinical trials. Collaborate with the multidisciplinary team to create and/or update patient’s plan of care that is feasible and understandable to the patient, family, and caregivers. Develop and deliver patient education. Use technologies such as EMR and My Chart to enhance communication among patients and health care providers. Develop and utilize documentation tools to track encounters of the Navigator with patients, families, and caregivers. Demonstrate leadership by motivating, educating, delegating, and mentoring peers while effectively managing patient care. Report outcomes of the Patient Navigation program to appropriate leaders and team members. Be willing to travel and work at other offices when needed. Perform other duties as assigned. Professional Development Identify own learning needs and areas of professional growth. Assume responsibility for personal professional growth and development. Attend educational offerings that promote professional development. Support self and peers in professional growth and development using resources within and outside the organization. Act as a resource, educator, and preceptor for staff and nursing students. Complete all required education to maintain competency for the specific patient population and regulatory agencies, including on‑time completion of annual education. Submit required tests and paperwork in a timely manner without management intervention. Participate in self and peer review activities, including positive recognition of peers and staff. Prepare for certification related to Clinical Navigator or specific patient population when eligible. Participate in chosen professional organizations, conferences, and research as appropriate. Evaluate nurse‑sensitive outcomes and participate in performance improvement initiatives. Qualifications Knowledge and Skills : Demonstrate knowledge, abilities, and skills to provide age‑and culturally specific patient care and education; function effectively in a busy setting with complex needs; communicate verbally and in writing with peers, physicians, patients, families, caregivers, and team members; use appropriate communication channels; maintain patient confidentiality (HIPAA); prioritize and perform multiple tasks; exhibit self‑motivation, flexibility, creativity, leadership, and presentation skills; apply critical thinking and problem resolution; adapt to change; maintain competence through continuing professional growth. Education : Graduate of an accredited school of nursing. BSN strongly preferred. Equivalent combination of nursing education and RN experience may be considered. Additional training in nurse navigation or oncology certification preferred. Years of Experience : Five years. Licenses & Certifications Current Registered Nurse license or interim permit in the State of Ohio or Kentucky. BLS Certification required.
