Our system of care includes a truly integrated team with the most talented, experienced, and compassionate caregivers and physicians, backed by the latest cutting-edge technology.The Community Health Navigator is responsible for providing outreach education, health assessments and community navigation services to identify, recruit, enroll and screen qualified program participants. The position is grant-funded through the Department of Public Health's CT Early Detection & Prevention Program (CEDPP). The position is contingent upon continued funding. Responsibilities include but, are not limited to:Provide health education & outreach activities to individuals in eligibility priority populationsCollaborate with local community agencies to identify potential underserved participantsEnroll eligible participants into the CT Early Detection & Prevention Program (CEDPP)Assess eligible participants for barriers preventing participation in early detection screening and prevention servicesComplete all required assessments, screenings and documentationWork closely with medical providers and clinical personnel to ensure that clients have integrated, comprehensive, coordinated healthcareAttend all required program meetings, DPH provider meetings and training sessionsDisplay and uphold HHC core values of dignity, compassion, service excellence, community and integrity. Consistently demonstrates caring for patients, for one another, and for the organization they are part of, and contributes to building trust, pride and camaraderieQualificationsEDUCATION: Minimum High School Diploma or equivalent, BS or BA in Social Services or related field strongly preferredEXPERIENCE 2-3 years of experience in a medical setting preferred.KNOWLEDGE, SKILL REQUIREMENTS, AND DESIRED ATTRIBUTESBilingual English/Spanish highly preferred.Excellent verbal and written communication skills a must.Valid driver's license and reliable transportation required.Basic clerical and personal computer skills, including Microsoft Office suite are required.Flexibility in work hours including some availability evenings and weekends when needed.Ability to work independently and communicate effectively via phone, video conferencing and in person a must.Ability to manage multiple priorities and ensure all grant program requirements are followed.We take great care of careers.
